Influencing Factors on Health Related to Quality of Life in Hypertension Patients

ABSTRACT

생활양식의 변화와 인구 노령화 등에 따라 만성질환 환자 가 증가하고 있으며, 2007년 국민건강영양조사에 따르면 우 리나라 30세 이상 고혈압의 유병률은 24.9% (남 26.8%, 여자 22.1%)를 차지하였다(Ministry of Health, 2007). 고혈압은 연령이 증가할수록 유병률과 발생률이 크게 증가하여 60세 이상 유병률이 남자는 42.3%, 여자는 49.1%로 조사되었다 (Ministry of Health, 2007). 그리고 고혈압은 뇌혈관 질환 및 심혈관계 질환을 야기하는 독립적인 위험 인자이며 사망 률 또한 2위를 차지하고 있다(Ministry of Health, 2007). 따라서 고혈압을 예방 및 관리하기 위해서 규칙적 운동, 올 바른 식이 습관 등의 건강한 생활 습관 실천의 중요성이 강조 되고 있으며, 약물 요법을 포함한 식이요법, 운동요법, 스트 레스관리, 금연 및 체중조절 등의 지속적 이행을 통하여 합병 증을 예방하거나 지연시킬 수 있다(Son & Yoo, 2004). 그러 나 환자의 대부분은 고혈압을 적절히 관리하지 못하고 있는 실정이다(Choi, 2008).

The purpose of this study was to identify the factors influencing health related to quality of life with hypertension patients. Methods: The subjects of this study were 409 hypertensive patients. Data were collected from March 1st to April 30th, 2008. A questionnaire consisting of exercise barrier, Psychosocial Wellbeing Index (PWI), EuroQol 5D (EQ- 5D) was given. The collected data were analyzed with the SPSS program which was used for descriptive statistics, univariate and multivariate analysis. Results: There was a significant relationship between exercise barrier (t=3.57, p=.000), psychosocial wellbeing (F=29.96, p=.000) and health related to quality of life. According to the result of multiple regression, the identified significant factors were an exercise barrier (F=7.09, p=.000) and psychosocial wellbeing (F=21.5, p= .000) with health related to quality of life. Conclusion: Hypertension patients experienced exercise barrier and psychosocial distress which led to a negative effect on health related to quality of life. Therefore it is needed to encourage motivation of lasting of exercise compliance and relieving of psychosocial distress for better health promotion and high quality of life.
